Student Equipment Loan Agreement
St. Patrick's Primary School is lending devices to students for the purpose of accessing remote learning during the COVID-19 Pandemic. The following applies to students who receive equipment on loan.
By signing this form, I agree to keep all equipment loaned in good condition. I also agree to return all items promptly once on-site learning recommences. I will return the computer to the office in person.
I understand I may be responsible for replacement costs if equipment is lost, damaged, destroyed or stolen. If I fail to return the borrowed items in good condition , the cost of repair or replacement may be added to my school fees account.
My responsibility as a Parent/ or Carer also extends to:
- Reading and agreeing to the school’s Information, Digital Technology Policy and discussing it with my child/ren so we both have a clear understanding of responsibilities for using digital technology to learn.
- Ensuring the Digital Technology Acceptable Use and Cybersafety Agreement is signed by both myself & my child/ren before returning it to the school.
- Ensuring my child/ren abide by eSafe practices at all times (Refer to https://www.esafety.gov.au/parents)

My signature acknowledges that I have received a copy of this agreement and understand our family responsibilities for the care and use of all the above listed items (devices).
I have read this contract and understand the conditions of the agreement.
